The following Model of Care (MOC) Training is designed to educate Healthcare Providers (HCP) on the structure, processes, and care management systems of Complex Care Management (CCM), Special Needs Plans (SNPs) and Cal MediConnect (CMC) which provide coordinated care to members with special needs described in MOC.
Once completed, you will receive a certificate of completion to verify you have finished the training.
